The malware cleanup may be completed quickly, but search engines need time to recrawl the site and remove old spam URLs. The index recovery period depends on the number of URLs, crawl rate, response codes, and how long the infection existed.
Usually not as the first step. Blocking can prevent search engines from seeing that spam URLs now return 404 or 410, which may slow removal. The correct response depends on how the malicious URLs are generated.
Google retains indexed URLs until they are recrawled and reprocessed. Cleanup stops the source; accurate status codes, clean sitemaps, removal tools where appropriate, and time complete the search recovery.
